We accept most major insurance plans and are in-network for many insurance companies, and we also offer payment plans for various oral procedures.

INFORMATION
Payment is expected at the time of your appointment for services. If you have dental insurance, we will provide an estimate of your co-payment and collect your portion at the time of your appointment. We accept cash and all major credit cards. We do not accept personal checks. We also offer Care Credit, which offers interest-free payment programs upon approval.
Your reserved time in our office is important. Treatment appointments will require a deposit when scheduling to secure this appointment time. This deposit will be applied to your estimated co-payment for services. However, if the appointment is broken without 2-business days notice, then the deposit is retained as a non-refundable missed appointment fee.
When reserving IV-Sedation appointments, the complete sedation fee is required at time of scheduling the appointment.
It is important to remember that your insurance coverage is a contract between you, your employer and your insurance company. Benefits and coverage vary significantly from plan to plan depending on what was agreed upon with your employer and the insurer.
Please keep in mind that the insurance is not designated to provide 100% benefit but rather to assist in the cost of dental care. Dental benefits should not be confused with dental care you need. You and Dr. Rubab Mirza determine your dental care collectively.
As a courtesy to our patients, we are happy to submit your dental claims for services rendered in our office. In order for us to do this, you must provide us with accurate and most up-to-date insurance information. We will verify your coverage and plan before your scheduled appointment, and using this information we will estimate the insurance portion and your co-payment. Please understand this is only an estimate and may not be what the insurance will pay. We will do our best to help you receive maximum benefits. Patients are responsible for and remaining balances for services received.
DENTAL INSURANCE
We are in-network for most insurance companies and we accept many major insurance plans. In most cases, dental insurance will not cover all of the expenses for a dental procedure, and more than likely, you will need to pay some amount out of pocket at the time of your appointment. Dental insurance helps reduce the overall out-of-pocket costs for you when you have procedures done. We file all insurance claims, track your existing claims, and follow up on delayed claims. We are committed to making your whole family dentist experience a pleasant one from beginning to end.
OFFICE POLICY
When you become a new patient, you will fill out a form that includes a section for your dental insurance. We ask that you fill this section out with your current insurance provider information so that we can verify coverage with them and determine how much of your dental care is covered. If you change insurance providers at any time, please keep us informed of the changes, including policy name changes, insurance company and address changes, or changes in your employment.
Whenever you come into our office for an appointment, bring your identification and insurance card with you, as this will speed up the verification process in our office. If you have questions regarding your dental coverage on a bill, contact our office and provide your insurance details so that we can connect with your insurance agency and sort out any issues. We file claims with your insurance provider within a few days after your procedure, and your insurance provider must pay their portion of the claim within 30 days of receipt. Any remaining balance after those 30 days must be paid by you.
If you do not have dental insurance, all payments must be paid in full at the time of an appointment. You can rest assured that we still take great pride in our work and in the care we provide to you. There are no shortcuts when it comes to providing the best dental care in Houston, and we accept all patients because we care about your oral health.
ACCEPTED INSURANCE & PAYMENT
We accept most insurance plans by in-network insurance providers, as well as certain employers’ insurance plans. Any payments can be made by check, cash, debit, and most major credit cards, whether for insurance covered procedures or procedures without insurance coverage. All services must be paid in full at the time of the service is performed. Any returned checks are subject to a processing fee determined by the bank and office.
